RECORD PREPARATION

Preparing records for a clinical review

Begin with the agreed review scope. Counsel and the consulting team should identify which facilities, providers, dates, and record types are relevant before any confidential records are transferred.

Request complete, legible records through authorized channels. Preserve original page references, distinguish duplicate documents, and note missing date ranges. Orders, assessments, medication administration, laboratory reports, handoff documentation, and discharge records can each answer different questions.

Use only the approved secure record workflow after it is activated for the engagement. Never attach patient records to the public inquiry form or ordinary email. Confirm the authority to disclose records and apply the minimum-necessary standard appropriate to the matter.

Provide a clear list of questions, the review deadline, and any known limitations. A record index and source-linked chronology help a reviewer trace each observation to its evidence.
